Congratulations on making the decision to work on your mental health! Here is the step-by-step process to get you enrolled as a new client.
Step-by-Step Guide to Getting Started
First, you need to let us know you plan to start therapy. To do this, feel free to use the form from the contact page to email from your personal account or call 331-588-5156.
Once you make contact, Cheryl will assist you in setting up your appointments.
Fill out Online Paperwork
Once an appointment has been made, you or your adult child (18 and up) will need to fill out the online paperwork. These directions can be found below along with the link to the patient portal. If using insurance, you will also need to take a picture of the front and back of your insurance card, with the DOB of the policyholder and the identified patient, and send it in a secure message via the portal. Renee from our billing department will then run your benefits and provide you with a printout of your benefits and help you understand your financial responsibility for co-pays. Payments are required at the time of services unless other agreements have been made in advance with Cheryl.
Register Through Patient Portal
Once you click on Cheryl’s “Portal” button, you will be taken to her patient portal website. You will then need to click on “Register as a new client” to create a new patient account.
In this section, you will enter your name (or your child’s), your date of birth (or your child’s), and your email address. You will then be sent an email that will contain an activation link. Once you click on that link, it will take you to a page where you will need to enter your date of birth (or your child’s). You will then be asked to create a username and password to complete the registration process. Please save this information somewhere safe. If you need help having it reset, please text or call Cheryl.
When you enter your portal, you will see your Dashboard page. This page is where you have access to all of the features of the portal including seeing your scheduled appointments, accessing statements, making payments, filling out online forms, and sending secure messages. (Cheryl Accepts Paypal, Zelle, Worldpay. No cash or checks)
Prior to your first session, you will need to complete your account set-up by completing your profile, providing your insurance information, and sending a picture of your insurance card. You can complete each task by clicking on the name of the task in the left column in your portal.
